THERE WILL BE A  CHARGE FOR COPYING OF MEDICAL RECORDS BY THE ADMINISTRATIVE STAFF OF OUR OFFICE—$1.00 PER PAGE PLUS APPLICABLE TAX AND HANDLING (IF APPLICABLE ).

THE RECORDS WILL BE PROVIDED TO PATIENT WITHIN 30 BUSINESS DAYS OF THE RECEIPT OF REQUEST  IN WRITING.
