We do NOT provide quotes without the benefit of physical exam/ or preliminary evaluation for out of town patients.
All inquiries re/ quotes will be redirected to our Appointment reservation/ Cancellation policy.

All costs will be provided only after consultation with the Doctor.

Appointment cancellation, no show policy

Consultation fee is $150, $5.00 plus surcharge is charged at the time you make an appointment the remaining balance of $145.00 plus surcharge is charged 24 hours prior to your confirmed appointment and is non-refundable. If you fail to cancel or reschedule your appointment- or if you notify us less than 24 hours (business days) prior to your appointment time- your credit card will be charged the full amount of the consultation fee plus surcharge.
