As the nose goes, it begins with: I wish I had a better nose to go with my attractive face. The men may say a new nose to go with my handsome face. Now the patients may not exactly say that but I know that’s what they’re thinking or they wouldn’t be here. Contrary to beliefs about plastic surgery, back in the day, to take action and change one’s appearance requires a significant amount of self-confidence and passion.

The  Primary Issue and the Story

This is the section of the consultation that we identify is why the patient is really here.
For me this begins a very exciting relationship and journey. The story is usually the same the attractive woman or handsome man just doesn’t embrace the nose that God gave them. Many people describe this aspect in very general in broad terms. But for me as the surgeon I am inspired by the single fact that the individual has strength, mindfulness, and knowledge, to address this issue and take the time to seek me out to improve and achieve their goals.
When did you first notice you didn’t like your nose – what is it about it that you don’t like – do you have a vision of how you would like to look. These are the topics we discussed initially, and I begin to get a feel for the direction we need to take.

Past Medical History

We must diverge for a moment during the consultation. This is after all real surgery. The patient is put to sleep undergoes significant anatomic intervention which then sets in motion the healing process. We must respect the intervention that is require and preoperatively to maximize safety, all systems must be evaluated and determined to be suitable to undergo such intervention.
The certified aesthetic consultant, is the lead person on this issue, gathering all necessary data from lab tests, chest x-ray, EKG and all past medical issues from associated specialists. This endeavor, that is the preoperative clearing process is essential to administering and delivering consistently safe service.

BACK TO BASICS..

At this point the patient and I and my aesthetic consultant who is always with me in the room begin to communicate and our plans begin to evolve.

THE PHYSICAL EXAMINATION..

At this time we move to the physical exam. Up until now my assistant and I in the patient have all been seated. My assistant positions the examination table and helps to get the patient comfortable and yet in the appropriate reclining position for
examination.
First with high intensity light I will examine the inside of the nose. This will assist me in determining the status of the septum, the mucosal lining  and the adjacent internal structures of the nose, which play a vital role in its function.
This internal exam is extremely important to provide the highest quality service to each patient. Next using a special caliper I will measure the length of the nose, the middle third with of the nose, the tip with, the tip to a alar groove on each side, the base with of the nose, the tip two lip distance, and the angle formed by the upper lip and the columella – that  the tissue that separates the two nostrils. If there is a hump it will be evaluated. I now have an objective set of facts that will change from surgery
and serve as a basis to objectively compare before and after results.
At this time I will give the patient a  mirror and will explain my vision for the new nose and the changes, required to achieve it. At this point the patient will usually look at me and not his or her head in agreement. We have now reached a meeting of the minds.

WISH PICs

Most all patients have a series of wish pics that demonstrate to me not so much what they want but defines their taste in noses in general. We all understand that the nose they get most likely will not exactly mimic the picture they show me. All are okay with this. I believe that reviewing our gallery of before-and-after’s, plus a thorough review of our gallery, allows me as the surgeon to achieve a clear understanding of the intended and expected result.
